Please complete one application per class / group (minimum of 15 people). You may request to do more than one activity during your visit.

NOTE: Spaces are limited. This form does not confirm your reservation. You will receive a confirmation by fax. We will contact you if changes are necessary.

We will try to accommodate requests, but we may need to suggest a different time or topic

3. Workshops

  • Ontario: grades K to 8.
  • Quebec: kindergarten, elementary, secondary cycle 1.
  • Maximum of 30 students per group.
  • Duration: 50 minutes (most workshops) or 75 minutes (Gross Anatomy workshop only).
  • Offered on Tuesdays, Wednesdays and Thursdays.
  • October 4, 2011 – May 17, 2012.
  • $6 per student and extra accompanying adult (taxes included).
